As we always expect at this time, malaria cases are spiking, but it's been much worse this month.

We are going to identify 2-3 local community health educators to be trained on how to administer a random diagnostic test (simple stick test for malaria) with correct clinical rationale and have them do sweeps of the communities with highest rates.

After utilizing the [Outreach EMR] records, we were able to identify parishes lacking health support, and not so surprising to anyone, they also have the highest number of malaria cases.

We're going to spend about a week training the educators before we send them out. In that time, I am going to develop a survey and assessment to give to them so they understand when it is appropriate when to test for malaria and also a survey to evaluate this, an outcome assessment, to provide follow-up on how to prevent severe cases from this point forward etc. We have paid for this with money saved from the prescribing audit using the [Outreach EMR] system.

“Dear Peter,

With over two decades of time spent practicing medicine in rural and urban areas throughout the United States as well as throughout the world I have seen many approaches to the challenge of medical record keeping for improving patient care and tracking purposes. In the Fall of 2018, in a rural region in eastern Uganda I had the opportunity to see and use a system donated to this FIMRC clinic where I spent 2 weeks. The system I had the opportunity to see and use was one developed, implemented and donated by the UK charity [now called Outreach EMR].

When I was initially shown the system my first concerns and surprise was the lack of bells and whistles. An hour later, when I felt completely comfortable and was actively using the system to see patients, order labs, review results, chart my visits, diagnosis and put in place my treatment plans, I was beginning to fall for this system. At the end of that long and busy day, having seen 140 patients with a very bright and educated local Ugandan nurse, I was convinced that the simplicity and lack of bells of whistles was the key to this system's success. This very simple, reliable and easy to learn system was perfectly suited for the context in which we were working and allowed for excellent efficient patient care and quick capture of diagnosis codes, medication usage, and the generation of a database that would allow us to predict pharmacy needs and changing disease prevalence by season, age, sex and region served by this clinic. The system developed by [Outreach EMR] is an easily customizable system with almost no learning curve that provides all the necessary functionality for delivering high quality care and keeping track of individual and population level information.”
